Job Description:
A company is looking for a Health Coach to support patient
medication adherence and lifestyle changes.
Key Responsibilities
Conduct outreach via phone, SMS, and video to engage and enroll
patients in the health program
Build rapport and establish trust with patients to motivate them
toward improved health behaviors
Monitor patient adherence and provide ongoing motivation and
support through regular check-ins
Required Qualifications
3+ years of experience in health education, patient engagement, or
a related healthcare role
Background in health coaching, public health, or community health
work
Strong interpersonal skills with the ability to engage in warm,
empathetic conversations
Experience conducting outreach via phone, SMS, and video
communication
Proficiency with database systems and mobile applications
